Device Sign-Out Agreement

STUDENT DEVICE SIGN OUT TERMS AND CONDITIONS
Chromebook/iPad/Laptop/Tablet/Device

Central Islip  Student Technology Device Sign-Out for Home and School Use. Please note there is a District imposed $25/year non-refundable technology fee associated with each device. Students are responsible for any repair costs above and beyond normal use incurred by the district (damages not covered by maintenance plan or lost/stolen devices). ANNUAL $25 PAYMENT SIGNIFIES AGREEMENT TO ALL TERMS AND CONDITIONS.

  • TECHNOLOGY FEE IS $25 PER YEAR PER DEVICE
  • Students will use the device for school use only and will refrain from personal uses. Web browsing is restricted for school use only and will be monitored and filtered by the district.
  • Devices must be returned at the end of each school year for summer maintenance and updating.  Devices are NOT available for summer programs or summer school.  The $25 fee covers September through June. 
  • Only Official District Accounts will have the ability to log in and use the Chromebook.  Personal Accounts will be restricted. 
  • All activity and internet use is tracked and monitored by the Central Islip School District for inappropriate and/or personal use. 
  • Students will not change any settings or attempt to bypass settings restrictions and will never attempt to disassemble or service the hardware. Keep the Device plugged in and charging whenever possible.
  • Students/Guardians are financially responsible for any damages, breakages, liquid damage, screen breakage, etc. either accidental or intentional beyond any maintenance plan repair coverages or not covered by the accidental damage agreement (IE Vandalism, intentional damage, etc.)
  • Parent/Student will bring the Device to the building Main Office for any support, maintenance, breakages, battery failures, screen damage or if the Device fails to operate as intended for support.  Loaner devicess are not available during support window of time.
  •  Never throw, slide, drop, or place object on top of the Device.
  • No Food or Liquids allowed around the Device.
  • Parents/Guardians acknowledge that students will always be supervised while using school technology. The district is not responsible for student’s exposure to internet browsing during home use.

  • Acknowledgement of $25 Annual Technology Fee which is non-refundable.  The technology fee will be used toward the Device maintenance plan to cover repairs of a normal wear and tear nature.
  • Upon the payment of the $25 fee each year, the parent/guardian and student are acknowledging and agree to return the Chromebook to the district in the same condition or agree to pay the replacement fee listed above. I further acknowledge we have internet access and WiFi in the home.
  • Parents/Guardians/Students are responsible for replacement chargers (45Watt USB-C) 
  •  REPLACEMENT COSTS: $400 - Chromebook | $30-Charger
ALL CHROMEBOOKS MUST BE RETURNED TO THE SCHOOL NO LATER THAN JUNE 30 EACH YEAR.
